- Gown Details Form
If you are missing any of these three pieces please let us know by contacting our customer service representative.
3. Fill Out Gown Details Form
It is very important that this form is filled out and returned with the dress, so that we know who the dress and accessories belong to, and how to best clean your dress.
4. Package Dress & Accessories
Place your dress and your accessories in the plastic bag then seal. Then, place the bag into the cardboard box. Tape the box up, and place the mailer sticker onto the box.
5. Drop off at Local FedEx
The last step is to drop off your package to your local FedEx location. You can expect your gown to arrive back between 3-4 weeks. Be sure to be home so that you can sign for your package, we don’t drop off dresses if the bride is not home. This is for the safety of the gown.

Common Questions about Filling Out Gown Details Form
Frequently Asked Questions
When you say, “date purchased” do you mean the day that the dress was bought, or the mailer kit was ordered?
List the date that your dress was purchased (roughly what year). This helps us determine the best method for cleaning your dress. Vintage dresses are going to be cleaned differently than modern dresses.
What if I don’t know what type of fabric it is?
There usually is a fabric care label on the inner lining on the dress along the skirt, or right next to the designer tag. If that tag is missing, our cleaning experts will try their best to guess the type of fabric your dress is made of. It is important that we know the fabric because not all cleaning methods work for different fabrics. For example, a silk dress will be cleaned entirely differently than a satin dress.
I lost my paperwork for the dress order, can i get it replaced?
Yes, we can email or snail mail you the paperwork and shipping labels. We can resend the mailer kit also. Just email or call us.
What do I put for the “Internal Use” Section?
Nothing 🙂 That is for the cleaners to fill out once it arrives at the shop.
